I support adults in Stockport that have a palliative diagnosis. My role can vary from being advisory to taking the leading role in providing care and support. We also ensure that we support the families and the friends and ensure that we recognise their caring role as well. And I also work across the hospital as well to support people with their discharges to make sure they've got everything that they need for going home. So my roles involve supporting all adults in Stockport who are in hospital and have got Care Act eligible needs to look at what they need for leaving hospital and more longer term support. We work alongside the NHS to improve people's outcomes for discharge planning. So we work in a person centred, strengths based role to empower people really to make choices over where they go next when they leave hospital. So we support them on that journey through health and social care services. I think a big thing for the people of Stockport and I think things have changed going forward is people having that choice of where they have their end of life care. And I think for me that's really important that we listen to people and people have that choice of whether they want to end their life at home or whether they want it to be in a hospice or in the hospital etc. But I think having that team support around me and working with other people means that we can actually give those people of Stockport that choice now. I think what's important to me is supporting people through the whole process of being in hospital. I find it fulfilling to support through that journey of what happens next to them. And so we empower people and support people through their illness and recovery. I just get so much satisfaction from doing the job. I just feel very supported by the colleagues that I have. I just feel like I have in the past almost nine years that I've been doing the job now that I have made some difference I hope to the people and families of Stockport.
